Montana SDMI HCBS Waiver

Trias Pathways Program for SDMI waiver participants

Trias Coaching is approved to provide Life Coach services for adults participating in Montana's Severe Disabling Mental Illness Home and Community-Based Services Waiver when the service is included in the member's authorized plan.

Person-centered goals

Services are connected to assessed needs and specific goals established through the member's case-management process.

Practical coaching support

Life coaching may provide motivation, emotional support, confidence-building, accountability, and support for developing independent living skills.

Coordinated access

Participation depends on waiver eligibility, assessed need, service authorization, the member's plan, provider availability, and coordination with the case-management team.

Clear boundaries

SDMI Life Coach services do not provide medical care, psychotherapy, diagnosis, emergency response, or clinical treatment and may not duplicate other authorized services.

Members, case managers, and referral partners may contact Trias Coaching to confirm current availability and the appropriate referral or authorization pathway.

A six-session pro bono coaching package for clarifying direction, strengthening whole-person alignment, and translating awareness into purposeful action.

A free annual Trias Coaching intensive digital group-coaching cohort offered in collaboration with International Coaching Week.

A workshop experience for learning the four MEPS systems, identifying areas that feel out of sync, exploring beliefs and mind-body patterns, and creating values-aligned next steps.
